Forms for Parents:
Once you enroll, there are a number of forms you will need to fill out, which are required by the State of California. These include two on you child’s health history and an emergency medical treatment authorization form. We also need a copy of their current immunization record.
Additionally, there are two advisory forms to read, including details on the caregiver background check process, and family childcare consumer awareness information.
